U.S. Host Company Forms and Documents

• Company Training Plan and Samples Training plan that must be filled out by U.S. host companies in order for participants to acquire J-1 Exchange Visitor visa.
• Host Company Manual Provides basic information about rules and regulations pertaining to trainees on J-1 Exchange Visitor visas.
• Host Company Intermediate Evaluation Form must be complete at the midpoint of training program.
• Host Company Final Evaluation Form Evaluation Form must be completed upon completion of training program.
